Specific Need
Our project is Caring for Our Neighbors Every Way, Every Day. Our team works together to provide clients with the highest level of care and compassion. Providing multiple services in one building available during business hours allows for an increased level of care coordination which in turn improves outcomes. Currently, we offer medical, mental health in partnership with Pawnee Mental Health, food assistance, and social assistance services. Our medical, and mental health services are available to uninsured individuals whose household income is at or below 200% of the Federal Poverty level. There are no charges for eligible patients/clients for in-house services.
Our Food Pantry serves those in need in in the county. We also provide Commodity Supplement Food Pantry program for Seniors, and the Emergency Food Assistance Program or Commodities to whose household income is at or below 130% of Federal Poverty level. Our Social Service assistance includes assistance with rent, utilities, helping displaced community members when there is a fire, disaster, or life-altering event, as well as finding resources that would help a person start or keep working.
Without funding CCM could not reach as many low-income people in need. Preventative and acute healthcare practices, chronic disease management, preventative, rent and housing assistance, and food distribution would not be possible to those in need. To continue these services, we need your support and generous monetary gifts.

In July 2001 Community Care Ministries opened its doors to the Pottawatomie County community seeing patients for acute issues. The clinic is a valuable organization that has been serving the Pottawatomie, Wabaunsee, and now the surrounding counties for 25 years. Focusing on our mission, we will continue to provide access to health and dental care which makes a critical difference to those patients and their families. That access provides peace of mind, economic security, and services to stay healthy.
Our Commitment
Community Care Ministries (CCM) is committed in our mission to provide optimum non-emergent primary care and limited mental health services to eligible individuals. All services provided in-house are of no charge to eligible clients and patients.
Medical Services
Community Care Ministries is a non-emergent primary care safety net clinic. Our non-emergent primary care services include but are not limited to:
- in-clinic provider visits
- prescription assistance programs
- chronic disease management
- foot/toenail clinics
- in-clinic nurse visits
- lab services (outside service)
Social Assistance
Many times, individuals find that they are in need of help with life issues. Unexpected occurrences can sometimes find us with more month than money. Below are a few things CCM can assist finding resources for:
- car repair: minor repairs or parts to help with transportation
- emergency utility bill assistance: limited assistance paying a utility bill
- emergency rent assistance: limited assistance paying rent one month
Food Assistance
CCM provides the following distribution options to assist with food shortages individuals and families may experience. Clients must complete CCM’s Application/Intake process for eligibility. There are no costs or fees for our food assistance distributions.
- CCM Food Pantry: our in-house food pantry option can be accessed once per month and is open to individuals or families. Must complete Application/Eligibility process for eligibility.
- Senior Box Commodities: This option is for those 60 years of age and older. Income, Application/Intake process requirements are required.
- EFAP USDA Commodities: Families and individuals can access this option every other month. Income, Application/Intake process requirements are required.
Application/Eligibility: Eligibility for CCM services individuals must:
- show their combined household income is at or below 200% of the Federal Poverty Level
- be uninsured (not Medicaid, Tri-Care, or other health care insurance)
- complete an Application/Intake process. Individuals must complete an application, a medical and a social screen. Income verification is required.
